Nursing support from the professionals
Horizon Support Services, Inc. prevents or limits adverse outcomes by providing Nursing Support Services that focuses on health and safety. All participants receive an Initial Nursing Assessment where health needs, health records and physician orders are reviewed. This initial assessment enables the Registered Nurse to determine if the participant requires and meets the criteria to receive Nursing Consultation Services.
- Nursing Consultation Services: The Registered Nurse provides recommendations to participant on how to access health needs and have them met in the community. Health care protocol is also developed or reviewed. A communication system used by the participant to communicate with their health care providers and direct support staff is developed or reviewed.
- Health Case Management Services: A Nursing Care Plan and protocols regarding the participant’s health needs is developed. The Registered Nurse provides recommendations to Health Care Providers and Direct Support staff on how to have participant’s health needs met in the community.
