Horizon Support Services, Inc. provides services in a participant’s home and in the community. These services are tailored around the individual’s Person Centered Plan developed specifically for them. The Person Centered Plan outlines the participant’s strengths, interests, goals, desires, health needs and important people in their lives. A strategic plan to accomplish these goals and services are discussed and agreed upon. Taking into consideration the strengths and weaknesses of our participants, they receive support and assistance from trained Direct Support Professionals to accomplish outcomes.
Horizon Support Services, Inc. teaches or develops skills of daily living, healthy living, money management and social skills. Integration into the community is achieved through involvement in social, cultural, art/craft and educational events.
